Your insurance card and a photo ID. If you have a co-pay or coinsurance or deductible, please bring a form of payment as well.
- Our administrative team is available from 8:00 am – 7:00 pm Monday through Thursday and 8:00 am – 4:30 pm on Friday.
- Psychotherapists are available Monday through Friday from 8 am – 8 pm. Please consult your individual provider for their specific hours.
Stone Arch Psychology and Health Services does not have a psychiatrist or nurse practitioner on staff to prescribe medications, but we do work with psychiatrists and nurse practitioners at other clinics and can make referrals to those providers.
Yes, all Stone Arch Psychology and Health Services clinicians offer Telehealth Therapy / Online Counseling sessions.
Each psychotherapist takes a different therapeutic approach in their work with clients. Common therapy modalities include Cognitive Behavioral Therapy (CBT), Dialectical Behavioral Therapy (DBT), Solution-Focused Therapy, Neuro-Linguistics Programming, Internal Family Systems, Hypnosis, and Mindfulness Meditation Techniques, among others. The psychotherapists at Stone Arch Psychology and Health Services provide a battery of psychological and cognitive tests that are designed to assess a wide range of psychological conditions. Our psychotherapists will work with you to schedule convenient, regular appointment times. As a courtesy to other clients needing appointments who are on our waiting list we request that you provide us with at least 24-hour notice if you need to reschedule or cancel an appointment. Clients who cancel an appointment less than 24 hours before a scheduled appointment will be charged $100 for every hour that was scheduled. Sometimes clients are unable to keep an appointment because of weather conditions, sudden illness or an unexpected personal emergency, in these cases the client is encouraged to contact us as soon as possible to explain the problem and in most cases the fee will be waived. If you do miss an appointment and don’t advise us, in most cases the late cancellation fee will be charged.
- Stone Arch Psychology and Health Services accepts credit cards, checks or cash for payment on the date of service.
- If you received a bill, you may either pay by phone with a credit card or send a check.
- Please remember co-payments are due at the time of service.
You will need to fill out and sign an Authorization for Release of Confidential Information form and return it to Stone Arch Psychology and Health Services.On the Authorization for Release of Confidential Information form you are able to identify the recipient to which you would like your medical records sent. Please include their complete address, phone number and fax number, and what type of records should be sent. Your psychotherapist will review the request and fax the information to the identified recipient. The psychotherapists at Stone Arch Psychology and Health Services welcome phone and email contact to answer questions about the services they provide and will address specific needs. The therapeutic relationship officially starts when a client comes in for an initial evaluation and signs a consent form for treatment. This form gives the psychotherapist permission to engage in the therapeutic relationship. The initial evaluation is a service that we bill for and can be paid by either the client or the client’s insurance provider.

- During our regular office hours one of our office administrators can leave a note for your psychotherapist, or they can transfer you to your psychotherapist’s confidential voicemail. Your psychotherapist will return your call as soon as they are able.
- If you are calling outside of our regular office hours, please listen to the automated message prompts and choose the psychotherapist with whom you wish to leave a message.
Each of the psychotherapists at Stone Arch Psychology and Health Services have an individual listing on our website. These listings include a description of their approach to therapy, their applied modalities and their philosophy of therapy. You will find these individual listings on our website under CLINICIANS. If you have additional questions, you can leave a voice mail for the psychotherapist of your choice, with your name and contact information; they will be happy to return your call.
Insurance providers and plans are unique. The best way to determine what your benefits are, and to find out if you have a co-pay, co-insurance or deductible is to call and ask your insurance provider, the number can often be found on the back of your insurance card. Your insurance provider may ask what “billing code” Stone Arch will be submitting the claim with, to specify the service. For your first appointment the “billing code” is 90791. For ongoing psychotherapy appointments the “billing code” is 90837.
Stone Arch Psychology and Health Services Self-Pay Rates:
- New Client Intake and Diagnostic Assessment: $250
- Individual Therapy Sessions: $175 per hour
- Diagnostic Testing Sessions: $175 per hour
We do not offer a sliding fee for new clients. If a current client has changes in their usual method of payment our practitioners are often willing to work with the client so that they will continue to receive affordable services.
